Getting Started - Generating Consent Forms

This outlines the steps to create, customize, and manage document templates for consent forms to ensure clients can sign them efficiently.

Navigate to General Consent Form

On the left-hand task bar under the "Settings" section, click on "Templates" and then "Document Templates"

From there, click on "General Consent Form"

To edit the consent form, click the "Manage Document Template" drop-down in the top right-hand corner, and then click "Edit"

This is where you can customize what information and wording will be on the consent form.

Use the check boxes to determine which information will appear on the consent form:

And the "Content" box is where you can put more information about what the consent form is for, such as authorization details, service details, or pricing:

Once the template is saved, a preview of the document will generate

Getting Consent Forms Signed: Online Bookings

There are a few way to easily get consent forms signed. The first way is to assign consent forms to certain services which will be signed during the online booking process.

To do this, first navigate to "Locations" under the "Settings" section on the left-hand side. If you have more than one location, choose your location:

Once you are in the desired location, click on the "Tab: Overview" drop-down, and then click on "Services"

Here you will see all of the services you have for the specific location

To assign a consent form to a specific service, simply click on the consent form icon for that service:

Then, click on the Document Template drop-down and choose which template you would like to assign to the service:

Whichever consent form you choose is what the client will be prompted to sign during the online booking process for that service.

Getting Consent Forms Signed: In-Person

If you need to have a consent form signed in-person at a visit, simply click on the visit in the Calendar:

Then, click on the "Documents" tab and then "Sign Form Now"

Choose which form you need signed:

Then, click the "Sign" button on the top right-hand corner:

Here your client will be able to sign the consent form on your phone or tablet:

Getting Consent Forms Signed: Via Link

If you would like to email or text a link to the client to have them sign the form simply click on the visit in the Calendar:

Then, click on the "Documents" tab and then "Sign Form Now"

Choose which form you need signed:

Then Click "Get Signature Link"

Click on the "Copy" icon so you can send the link to the consent form to client via text or email for signature:
