This outlines the steps to create and manage form templates to easily collect client information.

Understanding Form Templates

  • Form templates are general forms that you can send to clients or have on your website to collect general information.

  • They are not for scheduling actual visits.

Example: Appointment Request Forms

A common example of using Form Templates would be adding an Appointment Request Forms to your website:

When the client clicks on the "Submit An Appointment Request" button, they are directed to the appointment request form:

Any form that is created in "Form Templates", when submitted, will show up under "Forms Submissions"

  • For Example: When a client submits an Appointment Request Form from your website the form will also show up in the "Form Submissions" tab in SimpleDVM.

  • You will also receive an email and a push notification that a form was submitted

Managing Forms

To add a form or edit an existing form, go to "Forms Template" which is in the "Templates" drop-down under "Settings" and click "Add Form Template" or click on the form you would like to edit:

Click the "Manage Form Template" drop-down in the top right-hand corner, then click "Edit":

Here you can edit the content of the form such as the:

  • Title

  • Subtitle

  • Post Submit message

  • The information you would like to collect

  • The questions you would like the client to answer

Adding Questions to a Form

You can add multiple questions to any form in various formats (e.g., yes/no, dropdown).

To delete a question, click on the "X"

To add a question, click "+ Add Question"

Fill out the "Question Text" section and then choose from the "Question Type" drop-down

Here you can customize how the client is able to answer the question
